According to IC 20-34-4-5, all students must have proof of immunization prior to attending the first day of school. Please contact your physician or health clinic to have these records faxed to the school clinic at 317-861-2125, no later than October 23rd, 2017. After that date, your child will be excluded from school, and will not be permitted to attend school until proof of immunization is provided. If your child has not completed their full series of immunizations, you may contact the Hancock County Health Department at 317-477-1125 to schedule a no cost appointment.
